Type 2 Diabetes Mellitus

Patient’s Problem

A 48-year-old businessman, Mr. Ramesh, presented with a one-month history of increased thirst, frequent urination, and unexplained weight loss. He also reported occasional fatigue and blurred vision. He has a family history of diabetes and a lifestyle that includes a high-carb diet and smoking.

 

Diagnosis

Based on his classic symptoms and high-risk profile, a diagnosis of uncontrolled Type 2 Diabetes Mellitus was made.

Findings included:

  • High Blood Sugar: His fasting blood glucose was 162 mg/dL, and his HbA1c was 8.2%, both well above the normal range.
  • Physical Exam: He was overweight and showed signs of insulin resistance with acanthosis nigricans (darkening of skin on the neck).
  • Lab Tests: A urine test also showed signs of early kidney damage (microalbuminuria).

 

Treatment

The treatment plan focused on medication and significant lifestyle changes.

  • Lifestyle Modifications: He was advised to start a low-carb diet, exercise daily, and quit smoking.
  • Medication: He was started on Metformin to lower his blood sugar and a statin to manage his high cholesterol.
  • Monitoring: Regular blood glucose and HbA1c checks were scheduled to monitor his progress and prevent complications.

 

Result

The prognosis is good if Mr. Ramesh complies with the treatment plan. Proper control of his blood sugar will significantly reduce his high risk of developing severe complications like nerve damage, kidney disease, or a heart attack.
