Hashimoto’s Thyroiditis

Patient’s Problem

A 38-year-old school teacher, Mrs. Anita R. (name changed to protect identity), presented with a three-month history of fatigue and weight gain. She also complained of cold intolerance, constipation, dry skin, and hair thinning. She noticed a mild swelling in the front of her neck.

 

Diagnosis

Based on her symptoms and family history of hypothyroidism, the MD physician in Vadodara suspected an autoimmune thyroid disorder.

Findings included:
  • Physical Exam: A physical exam showed a slow heart rate and a firm, enlarged thyroid gland (goiter).
  • Lab Tests: Her blood tests confirmed hypothyroidism with a very high TSH (12.5μIU/mL) and low Free T4 levels.
  • Autoantibodies: The diagnosis of Hashimoto’s was confirmed by the presence of high levels of anti-TPO and Anti-thyroglobulin antibodies.

 

Treatment

The treatment plan focused on replacing the thyroid hormone her body was no longer producing.

  • Medication: She was prescribed Levothyroxine once daily.
  • Monitoring: Her TSH and Free T4 levels will be checked every 6-8 weeks to adjust her medication dose until her levels are normal.
  • Lifestyle Advice: She was also advised on a balanced diet and regular exercise to manage her weight.

 

Result

The prognosis is excellent with lifelong treatment. Her symptoms, including fatigue and weight gain, are expected to improve as her thyroid hormone levels return to normal. Early treatment will also prevent long-term complications.
